Addendum to the Investment Management

Financial Planning Agreement

It is understood that we have retained Button Financial, Registered Investment Advisor, for on-going investment management and financial planning advice.

I (we) understand that Sally Jo Button, CFP ® is the sole owner and only financial planner at Button Financial. Her potential incapacity could interrupt effective money management of our accounts with SSG, the introducing firm for the clearing house, Pershing, LLC.

For the purposes of efficiently transitioning our accounts to another broker/dealer and clearing house, and finding another advisor to assist us with our financial planning, Sally Jo Button, has created a continuity plan which names Susan O'Grady CFP ®, as an agent under a Durable Power of Attorney.

By signing below, I (we) authorize Susan O'Grady, CFP ® to access our accounts on-line through Netxpro, communicate with SSG about the accounts, and act in Sally Jo Button's place, should she become incapacitated. We understand that Susan will be acting as an independent contractor to Button Financial under this contractual agreement.
